I recognize that these classes require physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved and I am fully aware that there may be risks and hazards unknown to me connected with participating in the activities. **I represent and warrant that I am physically fit capable and I have no medical condition which would prevent my full participation in the Program **In consideration of being permitted to participate in the Program, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the program. **In further consideration of being permitted to participate in the Program, I knowingly, voluntarily and expressly waive any claim I may have against the LEIMAY, Ximena Garnica and Shige Moriya, its employees and teachers, and other participants for any injury or damages that I may sustain as a result of participating in the program. **I, my heirs or legal representatives, forever release, waive, discharge and covenant negligence or other acts .**I have read the above release and waiver of liability and fully understand its contents. ONLINE/ONSITE PROGRAM MEDIA RELEASE:
By participating in online or onsite programming by LEIMAY, **I hereby irrevocably grant LEIMAY full unrestricted media rights and permission to record, broadcast, stream, post and otherwise copyright and use, reuse, publish, and republish photographic portraits, pictures of me, likenesses, voices, biographical materials, or videos in which I may be included intact or in part. **I further irrevocably grant to LEIMAY the right and license to use and exploit any printed material or video material in connection therewith, and relinquish any right that I may have to examine or approve the completed product or products, the advertising copies, or printed matter that may be used in conjunction therewith or the use to which it may be applied in perpetuity, in all formats and media, whether now existing or hereafter devised.
